log: model identity on transform lines - #21

transform-in debug line now carries providerID/model.id from the V2 context event. Same sweep as omp #78 / pi #156 / proxy #164: weak-model compress-rejection loops must be attributable from logs alone.

Same sweep as omp #78 / pi #156 / proxy #164: model identity absent from logs made weak-model loop diagnostics depend on forensics.
What
transform-indebug line (V2 context path) now carriesmodel: providerID/idfromevent.model(null when absent). Debug-gated, no behavior change.typecheck clean, 54/54.
