Description: Issue #51113 ("Root-cause the 71+ day conversation-transcript fetch failure blocking Copilot Session Insights") was closed completed on 2026-08-07 after PR #51195 merged, claiming to fix the fetcher so it captures *-conversation.txt transcripts for Copilot cloud-agent runs. Today's Copilot Session Insights report (discussion #53621, 2026-08-18) shows find ... -name "*-conversation.txt" still returns 0 files — the exact same symptom, now an ~83-day streak (first noted 2026-06-23), continuing straight through the "fix" date with no change in behavior. The auto-filed #53622 (missing_data) only reports today's single occurrence — it doesn't capture that this exact bug already went through a full root-cause-and-fix cycle that provably didn't work in production.

This is also relevant to two prior meta-issues about this exact accountability gap: #51807 and #51892 ("require verified-merged evidence before closing self-filed reliability issues") — both closed without the underlying enforcement apparently sticking, since this is happening again with the identical bug.
