Offer explicit fallback and continuation choices during setup #108

Implemented and reviewed in draft PR #109: #109.
The user's existing Codex sheet and bounded integration now enable all four supported fallback triggers; assignments and spending policy are unchanged, and continuation remains absent.
Candidate 1.10.3 passes 765 Bun tests, strict typecheck, plugin/skill validation, static invariants, documentation and maintenance checks, and GitHub CI on
a86941b. Copilot's case-numbering finding was fixed, replied to, and resolved. Seven live Claude Code turns cover first-run and rerun choices, broad/quota-only/disabled fallback persistence, independent continuation enable/disable, and byte-identical custom-policy preservation. Native marker probes, smoke candidates and separate reviewers passed, with independent file readbacks. The PR records the installed version and candidate-tree identity.This issue remains open because the required Codex live setup conversation and persistence test cannot start: Codex CLI 0.158.0 with ChatGPT authentication rejects
gpt-6.1-sol@highas unsupported. The same terminal outcome occurred after correcting the temporary plugin config override. No substitute model was used. The temporary candidate installation was removed and its failed test sessions archived; the personal plugin installation was preserved.Next step: exercise the exact candidate from a working Codex user surface, either after restoring availability of the configured parent or with an explicitly authorized test parent. Verify the recovery questions, confirmation and save/readback. Then make PR #109 ready, complete any remaining review, merge with the expected-head guard, publish v1.10.3, and verify issue closure. No merge, tag or release has occurred because the installed-parent gate remains incomplete.
The earlier Codex blocker was specific to the executable chosen for validation.
/opt/homebrew/bin/codexresolves to standalone Codex CLI 0.158.0. Repeating the exactgpt-6.1-sol/highrequest still returns HTTP 400: "The 'gpt-6.1-sol' model is not supported when using Codex with a ChatGPT account."/Applications/ChatGPT.app/Contents/Resources/codex-cli/CodexCLI.app/Contents/MacOS/codexis the desktop-bundled Codex CLI 0.159.0. The otherwise equivalent read-only marker request succeeds for the same model and effort. No model substitution, credential copying, account change, or personal CLI installation update was needed.The exact candidate 1.10.3 was installed separately again, with all 213 plugin files matching the reviewed packaged tree. A live setup invocation through the app's binary presented all three fallback modes and an independent continuation choice. Independent readback confirms both existing disposable targets are unchanged before confirmation. Persistence checks are now running.
Setup currently recommends broader fallback only for new sheets and does not require an explicit recovery choice on reruns. Existing users can keep quota-only behavior without being offered broader recovery.
Offer three explicit fallback modes on first setup and reruns: automatic backend recovery through saved chains, usage exhaustion only, and no automatic fallback. Offer bounded same-route continuation separately, preserve existing policy until the user accepts a change, and show both policies at confirmation. Add reproducible installed-parent cases for Codex and Claude Code.
Completion requires the packaged candidate passing repository checks and live setup verification in both parent harnesses before merge and release. The originating chat also authorizes enabling broader fallback in the user's existing Codex sheet while preserving assignments and spending policy.
