Add Codex metadata and agent skill evals

Summary

Adds Codex install metadata for Apify Agent Skills while keeping the existing Claude Code, Gemini, and Markdown skill formats intact.

What changed

• Add .codex-plugin/plugin.json for the existing skill collection
• Add repo-level Codex marketplace metadata in .agents/plugins/marketplace.json
• Document Codex plugin installation in the README
• Add a small human-review eval set for Ultimate Scraper, Actor development, and output-schema workflows
• Document an optional Telvine publishing path with metadata-only telemetry guidance
