Add a temporary minimal dynamic-only paddle.mm out_dtype path for CUDA 2-D BF16 inputs. Static/PIR paths fail closed to avoid legacy graph compatibility and fusion-pass corruption risks. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
