Order polish entries by band; settle at 10 tol/p - #109

The cluster hierarchy needs entries grouped by threshold band, not fully sorted, so a counting sort replaces sortperm (comparison sort only when the bands far outnumber the entries). The polish now stops when no cluster shifts by more than 10 tol/p: on well-connected supports the Newton steps leave the log scales accurate only to a few tol/p, and the tighter threshold took three passes there for shifts of ~1e-12.
