fix: make Socket.WebSocketConstructor effectful for per-connection setup

The constructor service now returns Effect<WebSocketLike, SocketError> and is invoked afresh on every connection attempt, enabling effectful per-connection setup (e.g. signing protocol tokens with rotating keys) and middleware layers over a base constructor. Failures surface as a SocketError at reader acquisition, like any failed dial.
