codexhost launcher: Codex Desktop did not start the codexhost Host chain before timeout

一直连接不上 让pi找了下原因 也没解决

- 系统：Windows 11 Pro 25H2，build 26200.9168
 - CodexHost：@codexhost/cli@0.9.1；Codex Desktop：商店版 OpenAI.Codex_26.915.4065.0（26.908 也复现）
 - 现象：Codex Desktop did not start the codexhost Host chain before timeout；trace 停在 controller: installing Renderer Session
 - 关键证据：真实 Desktop 进程环境里没有 CODEXHOST_* / CODEX_CLI_PATH，即 EnableDebugging 的 AppX 环境注入未生效；shim 从未被 spawn
 - 已排除：Defender 关闭、Developer Mode、用户级空 CODEX_CLI_PATH
