Issue
Now that the SDPi tooling has been updated to create both the sdpi-standard and sdpi-supplement documents, the content needs to be updated to reflect the intent. See issue #499 for additional background.
Proposed Change
sdpi-standard: Update document to remove all content that implies that it is a supplement that will be ultimately integrated into the TF document. This primarily involves updating the front matter to reflect the nature of this new "standalone" profile standard. The changes will be editorial only, including any note boxes in the document that reflect "supplement" language.
sdpi-supplement: Update content to reflect the normal IHE supplement template (the current "standard" content) but add the high-level outline (e.g., major section headings) that provide an overview of the content but point to where it is located in the sdpi-standard document.
